Es necesario la derivación precoz de los pacientes con Enfermedad Renal Crónica Avanzada (ERCA: FG ≤ 30 ml/min: Estadio IV de la ERC) para que estos sean educados sobre las diferentes modalidades de tratamiento sustitutivo (TRS) y exista tiempo suficiente para realizar un AV permanente y funcionante antes del inicio de la HD (Fuerza de Recomendación C). 2. Conservación de la red venosa de los Miembros Superiores (MMSS): 2. ¿ Se deben evitar las punciones y canulaciones en los MMSS para disminuir la incidencia de oclusiones y estenosis venosas (Fuerza de Recomendación B). 2. ¿ En caso de necesidad de puncionar o canalizar venas de los MMSS el dorso de la mano debe ser la localización de elección, debiéndose rotar los lugares de punción (Fuerza de Recomendación C)...

PATIENT EVALUATION AND PREPARATION PRIOR TO VASCULAR ACCESS (VA) PLACEMENT: 1. Early referral of patients with advanced chronic kidney disease (ACKD: GFR ≤ 30 ml/min: CKD stage 4) is necessary so that they are educated about the different modalities of kidney replacement therapy (KRT) and there is sufficient time to perform a permanent functioning VA before the initiation of HD (Strength of Recommendation C). 2.
